Payment is required through My School Bucks for each Fall or Spring season. Your coach will verify payment after you register.

Sign the Code of Conduct

The student and parent/guardian Code of Conduct form is available while completing the activity payment. It must be signed each season.

🔐 Step 1 – Sign In with Your School Account

Use your s.dcsdk12.org Google account to sign in. This confirms your identity and pre-fills your email.

🧪 Admin test mode: You can complete and validate the entire form, but no registration will be saved. Return to the teacher dashboard.

👤 Step 2 – Your Information

Choose either your US women's or US men's size. Heel-to-toe length is included for reference.
Enter exactly 12 hexadecimal characters. The page will format it as xx:xx:xx:xx:xx:xx.
Enter a valid MAC address using six pairs of letters/numbers, such as aa:bb:cc:dd:ee:ff.

Windows 10/11: Settings → Network → Properties → Physical address (MAC)
  Or: Open Command Prompt and run ipconfig /all

MacBook / Mac: System Preferences → Network → Advanced → Hardware tab

Chromebook: Click the time (bottom right) → Settings → Network → click your Wi-Fi → ⓘ → Hardware address

PS4 / PS5: Settings → Network → View Connection Status → MAC Address (Wi-Fi)

Xbox: Settings → General → Network Settings → Advanced Settings → Wireless MAC

Nintendo Switch: System Settings → Internet → Connection Status → MAC Address

Format: xx:xx:xx:xx:xx:xx. Colons or dashes are fine when pasting; the form will clean and format it for you.

🕹️ Step 3 – Games You Want to Play

Select all games you're interested in. You can sign up for multiple.

⚠️ Note: Some games cannot be played at the same time due to scheduling conflicts. Your coach will review sign-ups and discuss the 2026-2027 Season schedule with you before the season begins.
